The United States Medical Licensing Examination (USMLE)

To be certified by ECFMG, international Medical Graduates must, among other requirements, pass a medical science examination. Step 1 & Step 2 of the United States Medical Licensing Examinations (USMLE) are the Exams currently qualifies the ECFMG Certification.

STEP 1

It is the basic medical science Examinations which assesses what you understand and can apply important concepts of the sciences basic to the practice of medicine, which emphasis on principles and mechanisms underlying health, disease and modes of therapy.

STEP 2

It is the clinical science component which assesses whether you can apply medical knowledge and understanding the essentials of patient care under supervision and includes emphasis on Health promotion and disease prevention. The Exam is on a computer based format and online. You can apply Step1 and Step 2 at any time, provided you need the eligibility requirements of these exams.

Eligibility

The Eligibility requirements of Step1 & Step 2 differ depending on the candidate whether he/she are a medical school students or a medical school graduates

Medical School Students

To be Eligible for Step1 & Step2, you must be officially enrolled in a medical school located outside the United States and Canada that is listed in the International Medical Education Directory (IMED) of the foundation for advancement of international medical education and research (FAIMER), both at the time that you apply and at the time you take the exam. In addition, the ‘Graduation years’ in IMED for your Medical School must be listed as “Current” at the time you apply and taking the exam. Your dean or registrar must certify your current enrollment status on the application. This certification must be current; the official must have signed the application within four months of its receipt by ECFMG. As soon as you graduate and receive your medical degree, you must send two photocopies of your medical degree and one full face photograph to ECFMG. In addition to being currently enrolled as described above, to be eligible for Step2, you must be within 12 months of the completion of the full Didactic curriculum by the beginning of your eligibility period of Step2.

The United States Medical Licensing Examination (USMLE)

The United States Medical Licensing Examination is a single, 3 steps Exam for Medical Licensure in the United States. The USMLE provide a common system to evaluate the applicants for medical licensure. The United States Medical Licensing Examination is sponsored by Federation of State Medical Boards of the United States and the National Board of the Medical Examiners. You can take step1 & Step2 in either order, provided that you need the respective eligibility requirements for these exams. However before applying for step3, you must have received official notification that you have passed both step1 & Step2 (More information on USMLE website at www.USMLE.org)

Professional Linguistic Assessment Board (PLAB)

You will need to pass the PLAB Test before you can apply for registration in the UK.

Summary

The Test is in 2 Parts:

Part 1

It is a computer marked written examination consisting of extended matching questions and single best answer questions. Out of the 200 questions, the questions on images also be asked. The time is 3 hours. There may be 30% SBA Questions. The candidate can have an unlimited number of attempts but you must pass part 1 within 2 years of the date of your IELTS Certificate.

Part2

It is an objective Structured Clinical Examination (OSCE). It takes the form of 14 clinical scenarios or ‘stations’, rest station and one or more pilot stations run for statistical purposes, where the marks do not count towards your results. Each station lasts 5 minutes. You must pass part2 within three years of passing part1. You can have 4 attempts at part 2. If you fail at the 4th attempt you will have to retake IELTS (unless you are exempt) and both parts of the PLAB Test. You must be granted registration within three years of passing part2 of the test.

Entry Requirements of the PLAB Test

Before you applied to take PLAB Test you must have 1. An acceptable primary medical qualification (PMQ) All IMG’s must possess an acceptable PMQ in order to apply for registration. We define an acceptable PMQ as one which meets the following 5 criteria.

The primary medical Qualification must have :

Been awarded by an institution listed on the Avicenne Directory (formally the WHO Directory of the Medical School) or otherwise accepted by the GMC and be currently acceptable to the GMC (for further information please check the web page for acceptable primary medical qualification) For those on the Avicenne Directory, Been awarded by an institution that has a physical address included in the Avicenne Directory. Been Awarded After course of study comprising at least 5500 hours (or 4 years full time equivalent study). Not involved a course of study undertaken wholly or substantially outside the country that awarded the primary medical qualification Have not involved a course of study undertaken wholly or substantially by correspondence Most of the PMQs listed in the Avicenna Directory currently are acceptable for the purpose of registration. The non accepted Medical Qualifications are listed on the web page Applicants must ensure that these primary Medical Qualification is acceptable before applying to sit PLAB. Should it later be confirm that an applicant’s Primary Medical Qualification does not allow them to be registered, the GMC will not acceptable any responsibility for any costs incurred or losses that results from the candidate’s decision to sit PLAB. If you are from outside the European Economic Area (EEA), you need satisfactory scores in the international English Language Testing System (IELTS) Test. These are the minimum scores you must achieve: Overall 7, speaking 7, listening 6, academic reading 6, academic writing 6. The general modules are not acceptable. The certificate is valid for 2 years. The British Council runs the IELTS Test in over 100 countries and more information can be gathered from BC website or from the IELTS website. In certain circumstances you may be exempt from the IELTS Test. You should also have 12 months post graduates clinical Experience (Foundation year 1 training post / Internship post) from a teaching or other hospital approved by the medical registration authorities in the appropriate country. You can take the PLAB Test without this experience but you should bear mind that it is set at a level that assumes that you have it. If you do pass the test without this experience, you will have to seek employment in a foundation year 1 training post (the grade occupied new medical graduates).
